POSITION​ ​OVERVIEW
Good​ ​Measures​ ​combines​ ​the​ ​power​ ​of​ ​human​ ​touch​ ​with​ ​innovative​ ​technology​ ​to​ ​help
people​ ​achieve​ ​their​ ​nutrition​ ​and​ ​health​ ​goals.​ ​We​ ​are​ ​currently​ ​seeking​ ​a​ ​forward-thinking, adaptive,​ ​clinically​ ​experienced​ ​Certified​ ​Diabetes​ ​Educator​ ​(CDE)​ ​to​ ​join​ ​our​ ​team​ ​in​ ​a​ ​part-time or​ ​full-time​ ​capacity.

PRIMARY​ ​DUTIES​ ​&​ ​RESPONSIBILITIES

●  Provides​ ​diabetes​ ​education​ ​and​ ​counseling​ ​to​ ​moderate​ ​and​ ​high-risk​ ​patients​ ​using​ ​the

Good​ ​Measures​ ​platform​ ​and​ ​coaching​ ​model.

●  Assists​ ​patients​ ​in​ ​monitoring​ ​blood​ ​glucose​ ​for​ ​the​ ​purpose​ ​of​ ​medication​ ​control​ ​and

adjustment​ ​for​ ​both​ ​insulin​ ​and​ ​non-insulin​ ​dependent​ ​diabetics.

●  Assesses,​ ​monitors​ ​and​ ​implements​ ​treatment​ ​plans​ ​for​ ​patients​ ​at​ ​various​ ​stages​ ​of​ ​the

disease​ ​with​ ​a​ ​focus​ ​on​ ​the​ ​prevention​ ​of​ ​complications​ ​and​ ​management​ ​of​ ​comorbid

conditions.

●  Documents​ ​and​ ​communicates​ ​care​ ​plans​ ​in​ ​accordance​ ​with​ ​established​ ​standards​ ​of

practice.

●  Collaborates​ ​with​ ​team​ ​members​ ​to​ ​provide​ ​patient-centered​ ​care.

●  Assists​ ​in​ ​the​ ​development​ ​of​ ​patient​ ​educational​ ​materials​ ​and​ ​resources.

REQUIREMENTS/SKILLS​ ​NEEDED

●  Current​ ​Registered​ ​Nurse​ ​(RN)​ ​or​ ​Registered​ ​Dietitian​ ​(RDN)​ ​licensure​ ​in​ ​the​ ​state​ ​of​ ​New Hampshire​ ​and​ ​qualified​ ​to​ ​become​ ​licensed​ ​in​ ​multiple​ ​states.​ ​Complete​ ​annual​ ​education and​ ​certification​ ​requirements.

●  Certified​ ​Diabetes​ ​Educator​ ​(CDE)​ ​required

●  Minimum​ ​five​ ​years’​ ​clinical​ ​experience​ ​with​ ​diabetes​ ​management.

●  Comply​ ​with​ ​all​ ​policies​ ​and​ ​standards​ ​regarding​ ​patient​ ​confidentiality​ ​and​ ​ethical

business​ ​practices.

●  Available​ ​to​ ​work​ ​evenings,​ ​weekends​ ​and​ ​some​ ​holidays.

●  Proficient​ ​in​ ​using​ ​and​ ​applying​ ​technology​ ​to​ ​clinical​ ​practice​ ​and​ ​objectives.​ ​Able​ ​to​ ​learn

computer​ ​software/applications​ ​quickly.

●  Self-starter​ ​with​ ​the​ ​ability​ ​to​ ​self-manage​ ​in​ ​a​ ​fast-paced,​ ​call-center​ ​environment.

●  Excellent​ ​communication​ ​skills,​ ​both​ ​written​ ​and​ ​oral.

●  Conveys​ ​empathy​ ​and​ ​a​ ​commitment​ ​to​ ​patient-centered​ ​care.​ ​Desire​ ​to​ ​go​ ​the​ ​extra​ ​mile

to​ ​provide​ ​excellent​ ​customer​ ​service.

●  Detail​ ​oriented​ ​and​ ​a​ ​conceptual​ ​thinker.

●  Collaborative​ ​working​ ​style​ ​and​ ​culture;​ ​must​ ​be​ ​able​ ​to​ ​contribute​ ​in​ ​a​ ​group​ ​setting​ ​as

well​ ​as​ ​give​ ​and​ ​receive​ ​feedback.

●  Ability​ ​to​ ​manage​ ​multiple​ ​and​ ​simultaneous​ ​responsibilities​ ​and​ ​to​ ​prioritize​ ​scheduling​ ​of

work​ ​to​ ​complete​ ​assignments​ ​accurately​ ​and​ ​in​ ​a​ ​timely​ ​manner.
